Background

SI PPS and NPSI recognize the significant impact of social determinants of health (SDOH) on individual well-being and the overall health of the community. This funding opportunity seeks to support local organizations that are offering services and resources addressing these determinants, but facing challenges or seeing lesser impact due to funding barriers or limitations. Targeting crucial social and environmental factors that can contribute to disease prevention, chronic illness management, and overall health and wellness are of particular interest, as they will help to create and support a more equitable and healthier community on Staten Island.

The pivotal role of social determinants of health in shaping health outcomes is well-documented. Factors such as education, employment, housing, nutrition, and access to quality healthcare services directly influence an individual’s well-being and ability to lead a healthy life. By addressing these determinants comprehensively, in partnership and across sectors, we can tackle the root causes of health disparities and promote holistic approaches to healthcare.

In working to address social determinants, a diversity of approaches and stakeholders helps to expand impact and improve outcomes. SI PPS recognizes that a range of organizations and providers possess valuable expertise and approaches to effectively address social determinants of health, and NPSI aims to support their commitment to helping such CBOs build their capacity. This funding opportunity will build on existing efforts by SI PPS to assist CBOs with increasing resources available to the community, sustaining services and long-term operations that address social determinants in key communities, and begin building bridges between health/medical providers and CBOs. 

Based on available data, the greatest need for SDOH supports exists in North Shore communities (zip codes 10301, 10302, 10303, 10304, 103010).

Nonprofit Staten Island (NPSI) is available to lend support to all prospective/eventual applicants. The NPSI team will field questions regarding this RFP and work in close coordination with SI PPS to ensure any needed clarifications are made available.
